Licensed Practical Nurse

Department: Nursing
Location: Oswego, NY

Nascentia Health Options seeks a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our team and deliver safe, high quality nursing care to patients in their homes Monday through Friday, 8am-4:30pm, with one weekend per month required.

Performance Responsibilities and Standards:
1. Deliver safe, high quality nursing care in accordance with agency policies, procedures and existing regulations.


a. Follow the established nursing plan of care for each patient.
b. Report changes in medications, treatment and patient condition to
RN case manager or nursing supervisor in a timely manner.
c. Document accurately and completely as required.
d. Use equipment and supplies properly and efficiently.
e. Maintain a safe and orderly patient environment.
f. Consistently demonstrate the practice of proper body mechanics.
g. Report all accidents, incidents and injuries promptly and accurately.
h. Keep supervisor informed of problems as they arise.

2. Demonstrate knowledge of agency policies and procedures.

a. Adhere to quality assurance standards.
b. Maintain patient confidentiality.
c. Demonstrate knowledge and use of universal precautions.
d. Maintain positive customer relations with patients, families and staff.
e. Maintain competency in basic nursing skills as required by agency policy.
f. Perform all other duties as assigned.

Requirements:
1. Graduate of an accredited licensed practical nursing program with current New York State LPN registration.
2. Previous home care experience.
3. Competency in performing licensed practical nursing techniques and procedures.


Physical Requirements:
1. Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.
2. Ability to perform tasks involving physical activity including:

  • Pushing/pulling – Typical weight 50 pounds; Maximum weight 150 pounds
  • Lifting and carrying – Typical weight 25 pounds; Maximum weight 40 pounds
  • Patient transfers, extensive bending, standing, kneeling, walking/driving in various weather conditions, e.g. snow, rain, sleet, and going up and down stairs.

3. Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.

Competitive Salary
This is a per-visit position with an estimated annualized range of $58,500–$75,000+, based on visit volume*.

*Actual compensation will vary depending on the number of completed visits per pay period. A minimum average of 60 visits per pay period is required to maintain full-time status.
