Registered Nurse, Care Manager Team Lead

Location: Syracuse, NY

The Care Management Team Leader provides clinical support to care management staff, including supervisors, care managers, and the education department.

Performance Responsibilities and Standards:

  1. Serves as an expert consultant to Care Managers.
  2. Serves as a lead in the orientation of new personnel, mentors peers, and promotes collaborative teamwork.
  3. Through joint visits, mentors/precepts staff to the role of Care Manager, ensuring that staff meet all of the job role expectations safely and timely. You may be required to work across various settings, including community, facility, and home environments, to assist in coordinating care for plan members.
  4. Completes supervisory visits with care management staff to ensure competency in skill sets such as assessment of member services, coordination of member care, and coordination of community agencies per regulatory/agency guidelines under the direction of the Care Management Supervisor.
  5. Completes UAS-NY validation visits with care managers per DOH regulations as needed.
  6. Assists with employee reviews and performance appraisals of the care management staff under the direction of the Care Management Supervisor.
  7. Provides coverage for other Team Leaders during of time of absence.
  8. Completes care management visits with members per agency need.
  9. Oversees pre-enrollment applicants who are potential transfers back to community settings that have been assessed for MLTC needs and are awaiting finalized discharge plans and determination of care plan from long-term placement. Collaborates with appropriate facility discharge planners when members transition between settings.
  10. Works collaboratively with the Education Department in providing education to the Care Management team. Also verifies attendance and ensures that education is provided to those that are absent or need reinforcement.
  11. Participates in regular team meetings and activities.
  12. Participates in departmental and organizational committees and meetings as applicable.
  13. Maintains a working knowledge of, and adheres to applicable federal/state regulations including, but not limited to patient confidentiality, the release of information, and HIPAA.
  14. Interacts in a manner, that is professional, respectful, positive, helpful, and promotes trust and commitment to co-workers, and maintains appropriate professional boundaries.
  15. Attends agency in-services and educational programs and maintains professional growth and development.
  16. Participates in the quality improvement and utilization review activities of the organization.
  17. Clarifies issues identified by Care Management and actively addresses concerns promptly.
  18. Performs other related duties as requested.

Requirements:

  1. Strong organizational and time management skills.
  2. Ability to manage conflict, stress, and multiple simultaneous work demands in an effective, professional manner.
  3. Ability and willingness to self-motivate, prioritize, and be willing to change processes to improve effectiveness and efficiency. Adapts to changing patient or organizational priorities.
  4. Ability to make independent decisions by established policies and procedures. Decisions and problem-solving require a combination of analysis, evaluation, and interpretive thinking.
  5. Computer literate and working knowledge of basic computer skills and Microsoft Office programs.
  6. Ability to work with patients/families of all ages and in a variety of settings, including inpatient facilities and patients’ homes presenting diverse physical conditions and social/cultural environments.
  7. Excellent written, verbal, and listening communication skills.
  8. Listens to or acknowledges the viewpoints of others.
  9. Sends clear, consistent messages to members/family/staff.
  10. Utilizes varied communication styles to communicate to produce results.
  11. An understanding of Medicare/Medicaid regulations and entitlements
  12. Knowledge of chronic disease management
  13. Demonstrated ability to work with a multi-discipline team.

Qualifications:

  1. Licensed and currently registered to practice as a Registered Professional Nurse in New York State, with a minimum of 1-year medical surgical experience. A BSN with one year of home care case management experience is preferred.
  2. Valid New York State driver’s license.

Physical Requirements:

  1. Ability to drive to and from a variety of settings in varying weather conditions.
  2. Physical exam signed by a physician showing you are in good health and have the immunizations required by the NYS DOH.
  3. Standing, sitting, bending, walking, and driving (frequently entering and exiting vehicles throughout the day) are standard postures.
  4. Frequently uses a laptop computer to enter orders and obtain product information.
  5. Sensory requirements include speech, vision, smell, and hearing.
  6. Lifting requirements of up to 20 pounds frequently and up to 50 pounds occasionally
  7. Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids, or tissues.
