Registered Nurse, MCH/Pediatrics

Perfect opportunity for Registered Nurses (RNs) in Oneida County

The Pediatric Maternal Child Registered Nurse, Case Manager provides skilled nursing assessments and care to pediatric clients in Oneida County. The Pediatric Maternal Child Registered Nurse, Case Manager is responsible for providing comprehensive care coordination and case management services to a diverse population of clients, including premature infants, children with disabilities, newborns and prenatal and postpartum individuals who may be at risk for adverse health outcomes. The RN Case Manager is responsible for direct patient care, coordination of overall home care services, and documentation of care for agency clients in accordance with Quality Standards and under the guidance of a Clinical Supervisor.

Performance Responsibilities and Standards

  1. Ensure quality and safe delivery of home health services including (but not limited to): case managing of client caseloads, developing a plan of care based on client assessment and OASIS evaluation, developing a plan of care using the 'interdisciplinary' approach, health status, developmental needs, social determinants of health, and ensuring that all documentation meets quality standards.
  2. Completes all OASIS evaluations, documentation reflects nursing care and interactions, documentation is clear and concise and documentation is completed in a timely manner (meets organizational standards).
  3. Develop individualized care plans in collaboration with clients, families, and interdisciplinary team members.
  4. Coordination of care: Coordinate and facilitate access to healthcare services, community resources, and support systems for the clients and their families. Ensure continuity of care across settings and providers.
  5. Advocacy: Serve as an advocate for clients and their families. Monitoring and Evaluation: Monitor clients’ progress towards achieving health goals and outcomes. Evaluate the effectiveness of interventions and adjust care plans as needed.
  6. Responsible for supervision/oversight of paraprofessional staff relevant to patient care; supervise home health aides in performing designated tasks related to patient care and per the care plan, and communicate appropriate information to home health aide department.
  7. Ensure that all documentation meets quality standards:
    o Completes all OASIS evaluations (when applicable).
    o Documentation reflects nursing care, interventions, and outcomes.
    o The documentation is clear and concise.
    o Documentation is completed in a timely manner.
  8. Comply with all Nascentia Health policies & procedures and quality initiatives; maintain patient/employee confidentiality at all times, demonstrating knowledge and use of infection control, federal/state/local government rules and regulations and compliance to all agency policies/procedures.
  9. Responsible for Coordination of Care. Provide guidance and education in client care to other disciplines. Work in conjunction with Clinical Supervisor to efficiently manage patients using a team approach.
  10. Coordinate with internal and external resources to meet patient needs. Communicate client care issues to interdisciplinary team.
  11. Maintain established productivity expectation.
  12. Must adhere and be knowledgeable of all organizational quality improvement standards.
  13. Maintain and update clinical knowledge on an on-going basis. Understand and follow nurse code of ethics and standards of practice

Job Qualifications

  1. Current New York State registered professional nurse license (degree must be from an accredited school of nursing). BSN a plus.
  2. Current and valid driver's license and auto insurance, and a vehicle which can be used for work (we do have a Fleet Program for field clinicians).
  3. Self-directed and organized with the ability to work independently in a fast paced environment.
  4. Minimum of 2 years of clinical pediatric and/or Maternal Child experience
  5. Proficient computer skills (including knowledge of electronic medical records software) with ability to learn new software.
  6. Exceptional oral, written and verbal communication skills.
  7. Some knowledge of community resources and healthcare delivery systems for pediatric and maternal-child populations.

Physical Requirements

  • Ability to sit or stand at a desk 90% of the day
  • Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 40 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
  • Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to use telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain product information
  • Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids
