Registered Nurse, Mobile Acute Care Team Leader

**** THIS POSITION IS ELIGIBLE FOR A $15,000 SIGN-ON BONUS ***

As a Mobile Acute Care RN Team Leader, you will play a pivotal role in transforming the way healthcare is delivered, reaching patients wherever they are in need. You will begin by screening patients for program eligibility and providing comprehensive education to patients, families, and caregivers about the program. In addition, you will provide direct patient care, ensuring the highest quality standards and collaborating with a clinical supervisor to ensure the best possible outcomes.

Performance Responsibilities and Standards

  1. Champion Quality Care: Your commitment to excellence will be crucial in delivering safe and exceptional acute hospital care at home.
    • Be a Hospital Liaison: When not providing direct patient care, you will serve as a valuable liaison at the hospital, bridging the gap between the hospital and home settings. Your role will involve maintaining open lines of communication and ensuring a smooth transition of care. You will have the opportunity to:
      • Screen patients for program eligibility, identifying those who can benefit from our mobile acute care services.
      • Empower patients and their families by educating them about the program and setting realistic expectations.
      • Facilitate seamless communication among various departments, agencies, and a collaborative interdisciplinary team of clinicians.
      • Coordinate internal and external resources to meet the unique needs of each patient.
  2. Provide Compassionate Clinical Care: Your expertise and compassion will shine as you provide clinical care to assigned patients in the private home environment. You will:
    • Engage with patients through a minimum of two personalized nursing visits per day, remote patient monitoring and virtual visits to ensure continuous care and support.
    • Collaborate with medical providers at the hospital, ensuring clear and timely communication about changes in patient condition, routine clinical updates, and progress toward goals, leveraging your strong clinical judgment and ability to advocate for patients.
    • Administer medications and deliver specialized care as ordered, drawing upon your advanced assessment skills and proficiency in high-tech procedures and equipment use.
    • Serve on call on a rotating basis, demonstrating your commitment to being available for patient needs beyond scheduled visits and ensuring continuity of care.
  3. Lead and Inspire a Mobile Care Team: As a team leader, you will guide and inspire a dedicated group of mobile acute care nurses. Your role will involve:
    • Providing guidance, support, and mentorship to the team, fostering a collaborative and patient-centered care environment.
    • Assisting with scheduling and coordinating nursing visits to ensure optimal coverage and patient satisfaction.
    • Acting as a resource and point of contact for the team, addressing any concerns and facilitating ongoing professional development.
  4. Fulfill any additional responsibilities and tasks as assigned, contributing to the overall success and excellence of our mobile acute care program.
  5. Other duties as assigned.

Requirements

  1. Nursing: License and current registration as a Registered Professional Nurse in New York State. Education: Bachelor's degree from an approved school, acceptable to the New York State Department of Health. Master's degree preferred.
  2. Experience as a clinician in home and community-based services is highly valued.
  3. Ability to provide exceptional care to individuals in private homes, fostering a comforting and healing environment.
  4. Excellent written and verbal communication skills, enabling you to manage stressful situations and resolve conflicts effectively.
  5. Proficiency in utilizing technology and computer applications, including Microsoft Office or other word processing, spreadsheet, and database management tools.
  6. Strong organizational skills with the ability to prioritize tasks and manage multiple responsibilities effectively.
  7. Resourcefulness in problem-solving, coupled with the ability to motivate and educate others.
  8. Ability to work independently, multiple assignments, prioritize workload.
  9. Computer literate and working knowledge of basic computer skills and Microsoft Office programs.
  10. Strong communication skills person to person and by phone.
  11. Ability to learn organizations software system to perform job duties.

Physical Requirements

  • Ability to sit or stand at a desk 90% of the day
  • Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 40 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
  • Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to use telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain product information
  • Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids
