Medical Social Worker

Department: Nursing
Location: Syracuse, NY

The medical social worker helps medically compromised individuals and their families who have psychosocial issues that are impeding them from obtaining maximum benefits from the treatment plan. Responsible for identifying social and emotional factors that are impacting on client's progress, developing a treatment plan, and communicating the plan to the health care team. This role primarily Supports Oswego County, and Onondaga County

Performance Responsibilities and Standards:

1. Responsible for the delivery of quality and efficient social work services per Nascentia Health guidelines.

  • Use problem oriented psychosocial assessments to determine patient's needs.
  • Discusses total client care with the client and the client's care-giver.
  • Develop and implement treatment plan with established goals based on the psycho social assessment, medical status and input from the client's family and/or care giver.
  • Assess client progress on an ongoing basis; revise care plan accordingly.
  • Accurately document client assessments, goals, plans and progress.
  • Utilize interdisciplinary approach to provide coordinated client care. Communicate clearly and promptly with staff regarding patient problems.
  • Notify Clinical Supervisor of any changes or developments in complex or problem cases.

2. Provide instruction and support to staff and other home care service providers.

  • Provide information and direction regarding appropriate community services and options for care to staff members.
  • Participate in interagency and hospital conferences as scheduled.
  • Assist staff in assessing client decision-making capability and other psychosocial concerns.
  • Serve as a liaison between community agencies and Nascentia Health.

3. Understand and adhere to agency policies and procedures.

  • Maintain client confidentiality.
  • Demonstrate knowledge of infectious diseases and use of universal precautions.
  • Demonstrate knowledge of federal, state and local rules and policies.
  • Adhere to uniform policy.
  • Maintain clinical records and reports on a timely basis in accordance with agency quality assurance standards.

4. Personal Growth and Development

  • Participate in annual evaluation with manager including a self-evaluation with personal goals.
  • Participate in agency and community in-services as scheduled.
  • Maintain acceptable clinical knowledge in areas of clinical interest through educational workshops and courses.

5. Performs all other duties as assigned

Requirements:

  • LMSW in Social Work required, and 1-year experience in healthcare setting.
  • Current New York State licensure. (CSW).
  • Current New York State driver's license and automobile insurance.
  • Reliable automobile.

Physical Requirements:

  • Speaking/hearing ability sufficient to communicate verbally both in person and on the telephone.
  • Ability to drive in varied types of weather conditions.
  • Ability to walk up and down stairs, and through rain, sleet and snow.
  • Requires varied use of bending, kneeling, sitting, standing and walking.
  • Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
