Clinical Supervisor

Location: Syracuse, NY

The Clinical Supervisor is responsible for overseeing the delivery and coordination of high-quality, compliant patient care and services within the Certified Home Health Agency (CHHA). This role includes supervising clinical staff, ensuring adherence to policies and procedures, participating in new staff orientation, supporting ongoing professional development, and facilitating accurate and timely data submission. Additionally, the Clinical Supervisor collaborates in clinical operations development and provides support during joint field visits. This position is full-time Monday-Friday, 8am-4:30pm, and reports to our Syracuse, NY headquarters.

Performance Responsibilities and Standards:

1. Quality and Safety of Patient Care

  • Ensure timely, safe, and compliant delivery of home health services, providing direct care if needed.
  • Maintain effective communication channels between caregivers, families, and patients, facilitating interdisciplinary collaboration and providing feedback to support effective case management.
  • Review client records, medical orders, and other documentation to confirm the quality and safety of care.
  • Serve as a leader for the interdisciplinary care team, modeling standards of professional practice.
  • Address complaints, grievances, and conflicts in collaboration with the Associate Director, resolving them promptly and effectively.
  • Investigate and report suspected cases of client neglect, as required by agency protocols and in consultation with the VP of Patient Services.
  • Participate in administrative on-call rotation as needed to support the agency.

2. Clinical Staff Supervision and Development

  • Conduct regular, clear communication with clinical staff through both verbal and written channels.
  • Perform joint visits with staff in patient homes to observe and guide patient care practices.
  • Ensure that staff comply with agency performance expectations, providing constructive feedback as needed.
  • Offer ongoing training and professional development to clinical staff to maintain high standards of care.
  • Evaluate staff performance systematically, providing feedback in a timely manner.
  • Support staff accountability in achieving their case management responsibilities.

3. Resource and Compliance Management

  • Serve as a resource to clinical staff, providing guidance on agency policies, practices, and procedures.
  • Demonstrate a thorough understanding of community resources, guiding staff in making appropriate referrals.
  • Ensure that all staff understand and comply with New York State and federal home care regulations, including patient rights and privacy laws.

4. Professional Growth and Team Collaboration

  • Actively manage, facilitate, and attend team meetings, promoting a positive and productive work environment.
  • Collaborate effectively with internal teams, managers, and external community resources to enhance patient care.

5. Other Duties

  • Perform other related duties as assigned to support agency operations and patient care.

Requirements:

  • Licensure: Current New York State Registered Nurse (RN) license in good standing.
  • Education: Bachelor’s Degree in nursing or a related field from a program approved by the New York State Department of Health and Two years Home health experience Or RN with four years home health experience and 6 credit hours or the equivalent of training / education in public health principles of management.
  • Experience: Clinical experience in a home health agency setting for a minimum of two years.
  • Transportation: Valid New York State driver's license, current automobile insurance, and access to a reliable vehicle for work purposes.

Physical Requirements:

  • Communication: Must have the capacity for effective written and verbal communication and be able to comprehend and relay information clearly.

  • Physical Activity:

    • Ability to push/pull up to 50 pounds; maximum 150 pounds occasionally.

    • Ability to lift/carry up to 25 pounds; maximum 40 pounds occasionally.

    • Mobility: Ability to walk and drive in varied weather conditions.

  • Equipment Use: Proficient in using telecommunication and IT equipment for documentation and communication.

  • Bloodborne Pathogens Exposure Category I: This position entails tasks that may involve exposure or potential exposure to blood, body fluids, or tissues.
