ISNP Nurse Practitioner, Medical Provider

Nascentia Health is expanding our Institutional Special Needs Plan (ISNP) service to provide seamless, coordinated, and comprehensive care for our facility-based patients.

We recognize the unique contributions of Nurse Practitioners (NPs) to improving the overall health of their patients and we want you to be on our team!

We follow an NP led patient-centered, value-based care model and we are looking to add experienced NPs to our team as we work to transform healthcare for our members.

The ISNP Nurse Practitioner Medical Provider (NPMP) is a position that embraces a collaborative Team approach to enhance care. You will be supported by a great group of physicians and NP partners who are knowledgeable and extremely approachable. You will be overseeing member care as the NPMP, consulting regularly with the interdisciplinary staff and allied health providers to provide a seamless delivery of care and services in accordance with CMS guidelines.

As a key member of the clinical team, you will practice evidence-based medicine within your scope of practice, including: preventive screening and health promotion; diagnosis and treatment of a wide variety of common, acute, and complex problems; ordering and interpreting diagnostic tests; prescribing medications; referring to specialists and collaborating other providers as appropriate; providing basic emergency care if necessary; and providing education and resources to members, families, and staff.

Responsibilities

Clinical

  • Perform comprehensive assessments and document findings in a complete, concise manner that is compliant with Nascentia documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations
  • Deliver cost-effective, quality care to assigned members
  • Regularly evaluate the plan of care for cost effectiveness while meeting the needs of members, families and providers to decrease high costs, poor outcomes and unnecessary hospitalizations
  • Manage medical and behavioral, acute and chronic conditions effectively, and in collaboration with a Care Team providers or specialty providers.
  • Responsible for ensuring that all diagnoses are ICD10, coded accurately, and documented appropriately to support the diagnosis at that visit
  • The NPMP is responsible for ensuring that all quality elements are addressed and documented per Nascentia and CMS guidelines
  • The NPMP will perform scheduled medication reviews and post-hospitalization medication reconciliations
  • Facilitate agreement and implementation of the member’s plan of care by engaging the facility staff, families/responsible parties, primary and specialty care physicians
  • Evaluate the effectiveness, necessity and efficiency of the plan in conjunction with the Team, making revisions as needed
  • Utilizes evidence-based practice guidelines and protocols established by Nascentia and CMS
  • Must attend and complete all mandatory educational and training requirements
  • Must attend and provide evidence of population-based CME training annually

Care Coordination

  • Understand the Payer/Plan benefits, CCM associate policies, procedures and articulate them effectively to providers, members and key decision-makers
  • Assess the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of benefit events and communicate the process to providers and appropriate team members
  • Coordinate and communicate plan of care as members transition through different levels of care and care settings
  • Monitor the needs of members and families while facilitating any adjustments to the plan of care as situations and conditions change
  • Review orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the member’s needs and wishes
  • Regular and effective communication with Care Team members, family or key decision-makers, nursing staff and specialists
  • Actively promote and support the Nascentia Health ISNP program in assigned facilities by modeling professional excellence and partnering with key stakeholders (i e : internal sales function, provider relations, facility leader) to maintain and develop membership growth

Required Qualifications:

  • NPs: Graduate of an accredited master’s degree in nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP), Adult, Family or Gerontology Nurse Practitioners with preferred certification as ANP, FNP, or GNP
  • Active and unrestricted license in NYS
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire
  • Possess Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification prior to performing work
  • Working knowledge of EMR Software and excellent computer maneuverability.
  • Knowledge of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts and benefit systems
  • Access to reliable transportation that will enable you to travel to patient sites within a designated area
  • Ability to lift a 30-pound bag in and out of car and to navigate stairs

Preferred Qualifications:

  • 2+ years of hands-on post grad experience within Long Term Care or 2+ years of hands-on post grad experience as a Hospitalist managing older adult and complex patients. Applicants with more than 1 years’ experience in geriatrics or hospital medicine will be considered
  • Understanding of I-SNP MOC and I-SNP population.
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Palliative Care and End of Life Care including counseling on Advance Directives and MOLST preparations
  • MEDENT experience
  • Ability to develop and maintain positive customer relationships

Compensation & Benefits:

Competitive Salary [This position is an S08 exempt position with a min-max rate of: $111,018- $138,458/annually]

  • Full-time, depending on experience.
  • On-Call compensation based on rotation with flat rate

Standard shift:

  • Day shift with rounding before 12 pm
  • Administrative time embedded into schedule

Weekly schedule:

  • 5x8 with flexibility
