QA Performance Improvement Supervisor

Department: Quality Assurance
Location: Syracuse, NY

The Quality Assurance/Performance Improvement Supervisor is responsible for the direct implementation of the Quality Improvement Plan including the identification of care outcomes and the development of appropriate intervention action plans throughout the Certified Home Health Agency (CHHA), Licensed Home Care Services Agency (LHCSA).

Performance Responsibilities and Standards:

1. Assist in the development and implementation of the CHHA and LHCSA Quality Assurance Workplan.

2. Oversees a team of Quality Professionals.

3. Facilitate quarterly reviews of the programs for the organization and reporting of results to the Professional Advisory Committee.

4. Develop and implement a multi-disciplinary approach to quality improvement, including the identification of goals and related patient/member outcomes throughout all service lines.

5. Supervise, coordinate, and facilitate auditing and monitoring activities performed by quality assurance staff.

6. Facilitate communication between all departments and levels of staff

7. Coordinate, participate and report results of activities and outcomes to staff and management.

8. Assist in the development of agency policies and procedures including annual review of agency policy manuals.

9. Assist in the verification of staff compliance to established regulatory requirements for the Local, State, Federal, and Accrediting Body.

10. Assist in the investigation and resolution of complaints and incidents.

11. Collaborate with compliance and staff development personnel to ensure optimal efficiency and efficacy of services provided by the organization.

12. Presenting specific reports, as needed, that describe program outcomes and progress toward goals.

13. Performs all other duties as assigned.

Requirements:

  1. Current license as a Registered Nurse.
  2. Bachelor's degree from an approved school, acceptable to the New York State Department of Health, Master’s degree preferred.
  3. Two years-experience as a clinician in home and community-based services preferred.
  4. Proficiency in Microsoft Office software including Word, Excel, and PowerPoint (Access preferred).
  5. Certified Professional in Healthcare Quality (CPHQ) preferred.
  6. Current New York State driver's license and automobile insurance.
  7. OASIS certified preferred.

Physical Requirements:

  1. Speech, visual, and hearing ability sufficient to express and comprehend written and verbal communication
  2. Ability to sit or stand at a desk 90% of the day
  3. Frequent sitting, standing, walking
  4. Pushing, pulling, reaching, kneeling
  5. Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
